Covered Providers

Meriter Home Health accepts Medicare, Medicaid and most commercial insurance plans.

Medicare and Medicaid Coverage for Skilled Nursing and Therapy Services

Home health care is a covered benefit under both Medicare and Medicaid, and Meriter Home Health is a participating provider for both programs. In order to qualify for home care coverage under Medicare, a patient must:

  • Be homebound
  • Be under the care of a physician
  • Require skilled nursing services, physical therapy, occupational therapy or speech therapy on an intermittent (part-time) basis
  • Reside in a home or facility that does not perform skilled care (in other words, the patient must not be located in a nursing home or hospital)

Medicare and Medicaid Coverage for Home Medical Equipment

Medicare and Medicaid cover most DME/RT products and services. Please contact our Customer Service Representatives at 608-417-3700 for specific coverage criteria and guidelines.


Commercial Insurance Coverage

Most insurance companies base their level of home care coverage on the Medicare guidelines, and require that services be authorized prior to coverage. Meriter Home Health accepts referrals for home health services and medical equipment from the following insurance providers and coverage sources:

  • The Alliance
  • AETNA
  • Associates for Health Care (AHC) - PPO
  • Blue Cross & Blue Shield (United of Wisconsin)
  • Care Wisconsin
  • Health EOS
  • Humana Gold Choice
  • Physicians Plus Insurance Corporation
  • Unity
  • Wisconsin Physicians Service (WPS)

If you have another insurance provider, in most cases Meriter Home Health can work with your insurance provider to establish coverage. Please contact your insurance provider to determine network provider status and authorization requirements for HMO and commercial plans. Your insurance provider will be able to provide related deductible, coinsurance, out-of-pocket maximum values, and year-to-date values that will help to determine potential out of pocket expense.
