 As
the Madison area grows, there is an increased request for primary care services.
As part of Meriter's mission to serve the community, we currently offer two primary care
clinics to serve this need.
Meriter Medical Clinic-Middleton
opened in 2004, and is staffed by six physicians board-certified in internal
medicine.
Our newest clinic is Meriter Medical Clinic-McKee,
located near the intersection of Highways PD and M on the west side of Madison.
The clinic is currently staffed by three physicians board-certified in internal
medicine.

Transitioning Your Care to a Meriter Medical Clinic
If you would like to transition your care, you'll find helpful information in our
Transition of Care instructions.
Also, you'll need to send in an Authorization/ Consent for Release of Medical
Information form and a Patient Registration form. These forms can be found in
our Forms for New Patients section.
